Enovis Launches Expanded Knee Revision System

Wilmington, Delaware-based Enovis™ Corporation has expanded its EMPOWR Revision Knee™ system to include stackable tibial, femoral, and diaphyseal cones. The addition of cones, according to Enovis, provides surgeons more options for treating bone defects in complex revision cases.

According to the company, EMPOWR’s femoral and tibial stackable augments can be used both medially and laterally which, the company expects, will minimize excess inventory.

Louie Vogt, group president of Enovis’ Reconstructive Business Group, told OTW, “The introduction of the EMPOWR Revision Knee fills a current market need for more efficient revision options, especially with growing interest in outpatient revision knee procedures. With the addition of the EMPOWR Cones, surgeons are better able to tailor revision surgeries with a modernized workflow and no decision fatigue.”

“In complex revision total knee arthroplasty cases, orthopedic surgeons must consider several factors to produce a positive patient outcome, specifically, the impact of the failed primary TKA, including subsequent bone loss and any soft tissue damage, and then properly correct the knee to produce an aligned and stable construct with the best possible implant combinations for the patient.”

“While revision TKA has seen many technological innovations over the last several years, efficiency has often been an afterthought. However, recent innovations combine advanced implant technologies with an efficient instrumentation, surgical technique, and delivery model to ensure successful implantation of the new components and positive outcomes for the patient.”

Miminal Tray Additions for Revision TKA Cases

“We developed the EMPOWR Revision system and EMPOWR Cones to facilitate minimal tray additions in the OR with ream-only cone preparation in one instrument tray, and the ability to stack cones with a unique compression slot implant feature. These systems aim to provide the latest in implant technology and options to address complex bone loss through an effective and reproducible technique rooted in past learnings.”

OTW asked Vogt to elaborate on decision fatigue and how it factors in revision TKA cases. He said, “As in other complex surgeries, surgeons can often be overwhelmed with the abundance of options to address revision TKA.”

“Traditionally, having more options is ‘better,’ but recent trends in revision TKA have shifted to an efficient, reproducible technique regarding stemmable, augmentable tibial and femoral constructs with short cemented stems and cones to fill bone defects, provide appropriate alignment and stability, and result in great patient outcomes.”

“Legacy revision systems often provide so many options that they can create decision fatigue, sometimes causing the surgeon to default to their comfort zone and use the technique that has worked the best for most of their patients.”

“We designed the EMPOWR Revision system to be a cost-effective, reproducible, and efficient implant solution and surgical technique to address a majority of revision TKA defects with positive results. Surgeons report comfort using the EMPOWR Revision system and EMPOWR cones with a 1-tray, ream-only cones technique with proven implant technologies.”

This is a fascinating development. In my practice we've seen similar outcomes with the revised protocol. The key differentiator seems to be patient selection criteria. Has anyone else noticed the correlation with BMI thresholds?

Great point. I'd push back slightly on the conclusion, the sample size in the cited study is too small to draw population-level inferences. That said, the directional signal is compelling and worth a larger RCT.
