After achieving its 250th implantation roughly two months ago, Centinel Spine, LLC, based in West Chester, Pennsylvania, has now announced the completion of the 500th U.S. procedure with its latest FDA-approved total disc replacement system of prodisc cervical solutions—prodisc C Vivo and prodisc C SK.

“The new keel-less prodisc C Vivo device offers a combination of a well-designed anatomical endplate shape along with robust spikes that provide strong immediate fixation,” commented neurosurgeon Richard Fessler, M.D. from Rush University, Chicago, Illinois. “Additionally, the simple instrumentation and minimal implantation steps result in an extremely easy-to-use system and efficient surgical technique.”

According to the company, the  flat endplate design of the prodisc C SK device allows for “optimized implant positioning that allows surgeons to address individual patient anatomy—and a low-profile central keel that provides immediate fixation and enables a streamlined keel preparation technique.”

When OTW asked how they reached this milestone so soon after the 250-procedure mark, Centinel Spine CEO Steve Murray replied, “First and foremost, this milestone was achieved so quickly through the strong dedication, planning, and execution of the entire Centinel Spine team.”

“Beyond this, the rapid adoption of these new prodisc cervical implant technologies is primarily due to surgeons experiencing the value that choice provides them and their patients. The prodisc cervical portfolio is the only total disc replacement system providing the ability to match the disc to the specific needs of the patient and the surgeon. The prodisc cervical offering is unique in the total disc replacement market.”

As for the keel-less design, he added, “There are a few things that make prodisc C Vivo, our keel-less design, special. First is the fact that prodisc C Vivo has been in clinical use internationally since 2009 and is one of the most widely used and clinically proven cervical total disc replacement implants in the world. Second, the implant has an anatomically shaped domed endplate that is unique. This dome shape provides excellent anatomic fit in many patients, and surgical efficiency.”

“Third,” said Murray, “the implant incorporates specially designed lateral spikes that provide strong immediate fixation, proven to be equivalent to large-keeled implants. Last, the system incorporates optimized instrumentation along with a simple one-step insertion technique that is quite different from other disc systems that require additional slot-cutting steps for rails or fins.”

“Although Centinel Spine’s prodisc technologies are new to the U.S., these devices are built on prodisc’s proven motion mechanics, long track record, and strong clinical data. Also, all of the prodisc devices incorporate prodisc CORE technology, the basis behind the predictable clinical outcomes of the prodisc platform after 30 years and hundreds of thousands of implantations worldwide.”

This is a fascinating development. In my practice we've seen similar outcomes with the revised protocol. The key differentiator seems to be patient selection criteria. Has anyone else noticed the correlation with BMI thresholds?

Great point. I'd push back slightly on the conclusion, the sample size in the cited study is too small to draw population-level inferences. That said, the directional signal is compelling and worth a larger RCT.
