Swiss device manufacturer Medacta has teamed up with Danville, Pennsylvania-based Geisinger, announcing that their lifetime guarantee for total hip and knee replacement surgeries has been extended to include knee revision surgeries.

Speaking of this new accord, Medacta noted, “This guarantee covers revision surgeries performed for patients with previous knee replacements that have failed, even if the original surgeries were performed outside the Geisinger health system or with joints from other manufacturers.”

“Geisinger and Medacta will stand behind the full cost of care associated with eligible revision surgeries over a patient’s lifetime, as long as they remain in the care of a Geisinger provider and are a Geisinger Health Plan member.”

“Since we began this value-based journey in 2017 to provide lifetime comfort to patients around primary hips and knees, we have continued to refine and enhance our processes to enable us to take this bold step into the world of revision total joint arthroplasty—and we’ve only just begun,” said Michael Suk, M.D., J.D., chair of the Geisinger Musculoskeletal Institute.

Francesco Siccardi, CEO of Medacta International, told OTW, “We initiated the collaboration with Geisinger in 2018, launching the hip replacement program. The response we received was very positive and in March 2019, we announced a first-of-its-kind pilot program for a knee replacement lifetime guarantee as well.”

“Following the success of these projects and faithful to our standing as a global and reliable partner in the healthcare industry, it was natural for Medacta to expand the program to include revision surgeries. Medacta’s comprehensive educational path, superior instrumentation and product portfolio, combined with the outstanding expertise of Geisinger’s clinical teams, allowed us to take this next step with confidence. We are proud to be part of Geisinger’s effort to lead the industry in true value-based care.”

This is a fascinating development. In my practice we've seen similar outcomes with the revised protocol. The key differentiator seems to be patient selection criteria. Has anyone else noticed the correlation with BMI thresholds?

Great point. I'd push back slightly on the conclusion, the sample size in the cited study is too small to draw population-level inferences. That said, the directional signal is compelling and worth a larger RCT.

We implemented a similar approach last year. Early results are promising but we're still gathering 12-month follow-up data. Happy to share our protocol if anyone is interested.
